Fill in and submit form Ε.Σ.23, accompanied with all the relevant documents for the declaration of forfeiture of a registered trademark. 

Upon the submission of the form, the fee of three hundred and forty eight euros (€ 348) is paid, payable in cash, by cheque, through an account maintained with the Department of Registrar Companies and Intellectual Property or through bank transfer.
