Fill in and submit form Ε.Σ. 22, to waive the rights deriving from a registered trademark entirety or in respect of certain products and services, accompanied by:

• the  fee of forty-nine euros (€49) is paid , payable in cash, by cheque, through an account maintained with the Department of Registrar Companies and Intellectual Property or through bank transfer;
• the consent declaration of the trademark owner or a third party proprietor of the trademark in the event that a license has been granted or the rights of that trademark have been transferred to a third party.
